Description: In this topic, the user will learn how to edit care notes with goal tracking as an employee in the web portal. This topic will also review how to view completed care notes, view and edit pending care notes, and how to publish care notes after clocking out of the shift. A Care Note is automatically created by DCI when a punch is created for a client with active goals for that service code. Care Notes will only be viewable in the web portal when an employee has completed a punch entry for a shift by selecting Publish Care Notes and Clock Out or Clock Out without Publishing Care Notes.

Role Required: Employee with Client who has Care Management enabled

View, Edit & Publish Care Notes in the Web Portal

  1. Log in to personal profile
    1. Overdue care notes are visible on the dashboard in the Overdue Care Notes Widget
  2. Click Care Management on the main menu
  3. Click Care Notes on the submenu
    1. Click Plan of Care on the submenu to view the client's details and plan of care details
    2. Click Client Summary on the submenu to view clients with care management enabled
  4. Search for a care note by using the filters and clicking Search
    1. Change the Status filter to Select Status to view all Care Notes including published and pending
  5. Select the Care Note to edit or view by clicking anywhere in the row
  6. The Add Care Notes page will display
    1. View tasks completed within goal tracking during shift in the Plan of Care widget
    2. View notes and attachments by selecting a tab
  7. Click Add Results next to the task to view and edit in the Add Task Result form wizard
  8. Review the task details and click Next
  9. Complete the Task Result tab
    1. Click the Result dropdown and select Met or Not Met
    2. Answer the question if listed in the Answer field
      1. Questions are not attached to each task result
    3. Notes and Attachments are optional
    4. Click Save and Yes to confirm
  10. Complete steps 7-9 for each task listed in the Plan of Care widget
  11. Click Actions
  12. Select Publish from the drop-down menu to publish the Care Note
    1. Care Notes cannot be edited once published
  13. Click Yes in the confirmation alert window
