Description: In this topic, the user will learn about the custom fields Phone Type and Phone Carrier. These fields are for organizations that add EVV locations. If enabled, Phone Type and Phone Carrier may be selected when creating a program or profile with an EVV location, or they may be added to an existing program or profile.

Create Phone Carrier
Log in to profile
Select Settings from the main menu
Select Custom Drop Down from the submenu
Click Actions
Select Add New Custom Drop Down Item from the drop-down menu
Complete the Add New Custom Drop Down Item form wizard
Item Type: Select Phone Carrier
Item Name: Create phone carrier name (i.e., Verizon, ATT)
Item Status: Choose Active or Inactive
Click Save and Yes to confirm
Repeat as needed
Create Phone Type
Log in to profile
Select Settings from the main menu
Select Custom Drop Down from the submenu
Click Actions
Select Add New Custom Drop Down Item from the drop-down menu
Complete the Add New Custom Drop Down Item form wizard
Item Type: Select Phone Type
Item Name: Create phone type name (i.e., landline, mobile)
Item Status: Choose Active or Inactive
Click Save and Yes to confirm
Repeat as needed
